CHAMPVA vs TRICARE — Which Coverage Is Right for Your Family?

Healthcare coverage for military families after separation is one of the most important — and most confusing — benefits decisions veterans face. Two VA-related healthcare programs often cause confusion: TRICARE and CHAMPVA. They sound similar, serve overlapping populations, but work very differently. Understanding which applies to your family and how to maximize your coverage could save thousands in healthcare costs.

What Is TRICARE?

TRICARE is the healthcare program for active duty service members, National Guard and Reserve members, military retirees, and their eligible dependents. It’s administered by the Department of Defense and provides comprehensive healthcare coverage through a network of military treatment facilities and civilian providers.

Who qualifies for TRICARE:

  • Active duty service members (automatic enrollment)
  • Active duty family members
  • Military retirees with 20+ years of qualifying service
  • Retiree family members
  • National Guard and Reserve members (under certain activation conditions)
  • Surviving dependents of service members who died on active duty

Key point: TRICARE is for military retirees (20+ years) and their families, plus active duty. Veterans who separated before 20 years generally do NOT qualify for TRICARE — they’re eligible for VA healthcare, which is different.

TRICARE Plan Options

TRICARE Prime

  • HMO-style plan with a primary care manager
  • Lowest out-of-pocket costs
  • Must live near a military treatment facility or TRICARE Prime service area
  • Annual enrollment fee: $0 for active duty; retirees pay ~$350/year (individual) or ~$700 (family)
  • Copays: $0 for active duty; retirees pay $12–$30 per visit at civilian providers

TRICARE Select

  • PPO-style plan — see any TRICARE-authorized provider without referral
  • Higher out-of-pocket costs than Prime but more flexibility
  • Available in all areas
  • Annual enrollment fee: $0 for active duty; retirees pay ~$350–$700/year
  • Retirees pay 20–25% of allowable charges after deductible

TRICARE for Life

  • For military retirees age 65+ who have Medicare Part A and B
  • TRICARE acts as secondary payer after Medicare
  • Dramatically reduces out-of-pocket costs for Medicare-enrolled retirees
  • No enrollment fee beyond Medicare Part B premium

TRICARE Reserve Select

  • For National Guard and Reserve members not on active duty
  • Monthly premiums apply — approximately $46/month individual, $231/month family (2026)

What Is CHAMPVA?

CHAMPVA — Civilian Health and Medical Program of the Department of Veterans Affairs — is a VA healthcare program for the spouses and children of veterans who meet specific criteria related to disability or death.

CHAMPVA is NOT the same as TRICARE. They’re separate programs run by different departments.

Who qualifies for CHAMPVA:

  • Spouse or surviving spouse of a veteran with a permanent and total (100% P&T) disability rating
  • Surviving spouse of a veteran who died from a service-connected condition
  • Child of a veteran with a permanent and total disability rating
  • Child of a veteran who died from a service-connected condition
  • Surviving spouse of a service member who died in the line of duty (if not eligible for TRICARE)

Key point: CHAMPVA is specifically for dependents of veterans rated 100% permanent and total — not for veterans themselves. A veteran rated 100% P&T receives VA healthcare directly; their family members may receive CHAMPVA.

CHAMPVA Coverage Details

CHAMPVA provides comprehensive coverage similar to a private health insurance plan:

  • Inpatient hospital care
  • Outpatient medical and mental health services
  • Prescription medications (through the CHAMPVA Meds by Mail program)
  • Durable medical equipment
  • Skilled nursing facility care
  • Hospice care

CHAMPVA cost sharing:

  • Annual deductible: $50 per person ($100 per family maximum)
  • CHAMPVA pays: 75% of allowable charges after deductible
  • Beneficiary pays: 25% (cost share) plus any difference between allowable and actual charge
  • Annual catastrophic cap: $3,000 per family — after this, CHAMPVA pays 100%

There is no monthly premium for CHAMPVA — the VA charges nothing for enrollment. The only costs are the annual deductible and 25% cost share per visit.

Can You Have Both CHAMPVA and TRICARE?

You generally cannot receive both CHAMPVA and TRICARE simultaneously. If you’re eligible for TRICARE (as a military retiree’s dependent, for example), you’re not eligible for CHAMPVA. CHAMPVA is specifically for those who aren’t eligible for TRICARE.

However, CHAMPVA and Medicare can work together. CHAMPVA beneficiaries who have Medicare Part A and B receive coverage where CHAMPVA acts as secondary payer — covering costs Medicare doesn’t pay. This combination can nearly eliminate out-of-pocket medical expenses.

How to Apply for CHAMPVA

  1. Confirm the veteran’s 100% P&T rating is in place (you’ll need the rating letter)
  2. Complete VA Form 10-10d (Application for CHAMPVA Benefits)
  3. Include required documents: proof of marriage (spouse), proof of birth (children), the veteran’s rating letter, Medicare card (if applicable)
  4. Mail to: VHA Office of Community Care, CHAMPVA Eligibility, PO Box 469028, Denver, CO 80246-9028
  5. Processing time: 4–8 weeks

How to Apply for TRICARE

Military retirees and their families enroll through the Defense Enrollment Eligibility Reporting System (DEERS). Update your DEERS record and select your TRICARE plan through the TRICARE website or your regional contractor.

The Bottom Line

TRICARE is for military retirees (20+ years) and their families. CHAMPVA is for the dependents of veterans rated 100% permanent and total. If you’re a veteran rated 100% P&T, your spouse and dependent children may qualify for CHAMPVA — providing comprehensive healthcare coverage at minimal cost. Apply immediately if you haven’t — CHAMPVA coverage starts from the application date, not retroactively.
